A better indication of the current streaming quality would also help imo. About half of my users have had to be reminded to change the quality options, simply because they had no idea that they were streaming at 720p. What I need is my users to be easily aware of quality settings without me having to remind them each time they log into a new device/browser. I don't need "Maximum quality" / Direct Play to be the default setting. In their scenario they should have an easy way of upping the "default quality". I have around 25 users on my server, none of them have bandwidth caps and most of them have fiber (NA/EU). Your users are likely not alone in terms of bandwidth limitations, but you're probably not the majority. If your users want to limit their streaming quality even more, they also have to look for the 480p option. So even if my users have set it in their desktop app, they have to do it again on their tablet, TV or phone. Let's also not forget that the default streaming quality setting is per-device. The Problem It appears that the majority of Plex clients on my network were considering the server to be a remote server, and so were setting bandwidth restrictions on playback.
